Anthem is a youth group in Markham for students from Grade 7 to 10. We meet regularly 2-3 times every month on Sundays between 3-4:30 p.m. Orientation starts Sunday, September 19.

By registering at Anthem, you are agreed to observe and abide to the rules and policies set by the governing body of Calvary Logos Baptist Church. If you are under 16, parent/guardian's acknowledgement and consent is necessary. 

*Note*: Our program will be held online via Zoom for the remainder of 2021. There is a possibility our venue at Sir John A. McDonald Public School at 45 Murison Dr., Markham ON (Woodbine/Major Mackenzie) will be opened for 2022. More details will be confirmed and communicated to you.

I give permission to my child to participate in Anthem and Awana Club organized by Calvary Logos Baptist Church (CLBC) and I agree that in the event of an accident or injury, CLBC, its staff, mentors and volunteers are hereby released from any liability.  I also agree that in the event that my child conducts him/herself in a manner that is disruptive will receive appropriate warnings; yet if there is no improvement he/she might be banned from the Club. I understand, accept and agree that as a condition of my child’s participation in any activity organized by CLBC that I will truly save, defend and fully indemnify the said Church and said Club from and against all manner of actions, suits, claims and demands which may be brought against or made upon it and against all loss, cost damages or other expenses which it may sustain, suffer, incur or be liable for as a result of my child’s participation in any Club activity.

I also give permission to CLBC and it's staff, mentors and volunteers to communicate with my child directly via text, email, social media or equivalent.

I agree to follow all rules set forth by Calvary Logos Baptist Church (CLBC) and am responsible for my conduct at Anthem as a youth participant, and at Awana Club as a volunteering helper.  I fully accept all responsibilities of any accident or injury to myself while I am participating in the program.  I further agree that CLBC, its staff, mentors and volunteers will not be liable for any injuries to myself.  I understand, accept and agree that as a condition of my participation in any activity organized by CLBC that I will truly save, defend and fully indemnify the said Church and said Club from and against all manner of actions, suits, claims and demands which may be brought against or made upon it and against all loss, cost damages or other expenses which it may sustain, suffer, incur or be liable for as a result of my participation in any Club activity.

I hereby grant and convey unto CLBC all rights, title, and interest in any or all photographic images and video or audio recordings made by Anthem or the Awana Club for the purpose of celebration or promotion. I agree that I will not hold CLBC responsible for any harm that may arise from any authorized or unauthorized reproduction. I will let the organizer knows if I want to opt out my photo on social media posts.
