Mentor Application Form

Must be a regular attendant of Sunday Worship at CLBC
Must go through mandatory training in a self-learning environment that consists of 13 short videos (5-6mins each) with reflection questions provided in the training guide within a 4-week period
(OPTIONAL) Joining Mentoring Discussion Group via Zoom on Sunday June 14,21,28 & July 5 between 11:15-12:30
Be a born again Christian who has a growing relationship with Jesus Christ
Be baptized
Adheres to the following:

I am a born again Christian in a growing relationship with Jesus

I have been baptized

I agree with CLBC's Statement of Faith

I agree with CLBC's Discipleship Pathway in making disciples

I agree with CLBC's Core Values and Practices

In the event that you are paired up with a mentee that is a minor (under 18), you will be required to complete a vulnerability check that will be subsidized by the church. *

I am in agreement that I will complete a vulnerability check if I am paired up with a minor.

Expectations of a Mentor

We expect all mentors to:

Meet up with your mentee on a bi-weekly basis at minimum (meet at least once every 2 weeks)
Commit to meeting up with your mentee for a 3-month period
Check in with an accountability coach on a weekly basis to provide status updates

You can choose to meet with your mentee in a format that both of you are comfortable with (in-person, virtual, phone, etc.)

There will also be an optional monthly mentors forum that will provide a platform for mentors to share their experiences, to learn from one another and to encourage one another.
